Two Types of Coverage
Medical Insurance
- Treats diseases and injuries of the eye (infection, glaucoma, cataract, diabetes, trauma).
- Typically covers medically necessary testing and procedures.
Vision Insurance
- Covers glasses and contact lenses, plus basic screening exams.
- Does not cover refractive surgery (LASIK, SMILE, EVO ICL), presbyopic or premium lens upgrades, or cosmetic procedures.
Vision plans (VSP, EyeMed, Spectera) never cover elective refractive surgery.

What’s Usually Covered
- Treatment of eye disease and injury
- Emergency visits
- Cataract surgery with a standard lens
- One eye per day or a single testing session limit
What’s Not Covered
- LASIK, SMILE, EVO ICL
- Presbyopic or premium implants and other “upgrades”
- Cosmetic pterygium surgery
- Comfort add-ons (heat therapy, extra scans beyond medical necessity)
- Extended follow-up beyond the 90-day global period (cash packages may extend to one year)
Understanding Costs
Copay — due at the time of visit; cannot be waived.
Deductible — amount you pay before insurance pays.
Coinsurance — shared percentage after the deductible.
Billing Reality
- Clinics are required to collect copays, deductibles, and coinsurance.
- Some advanced procedures have no valid CPT code; a code existing ≠ coverage.
- Even with a code, “medical necessity” criteria may not be met (e.g., cosmetic pterygium), so the service is patient-pay.
- If you believe your plan covers a service, you may pay the clinic and request claim submission for possible reimbursement.
