This page is the data-deletion instructions URL for our Meta (Facebook) app KVI LACS. It also explains how to request deletion of personal data we may hold from other channels.
1. What the Facebook connection does
KVI LACS is a staff-only review tool. When connected, it allows authorized Khanna Vision Institute staff to read posts and public comments from our own Khanna Vision Facebook Page for internal review and quality purposes.
The app does not:
- Post on your behalf
- Reply to comments or messages as you
- Send messages to Page visitors
- Collect or build profiles about visitors to the Facebook Page for marketing
2. What is stored
- No Facebook post or comment text is stored by KVI LACS as a lasting archive of Page content.
- Page access uses a short-lived encrypted connection that expires within 15 minutes and is then purged.
- Staff can disconnect the connection at any time from our internal tools and/or by removing the app in Facebook (see below).
3. How to remove the KVI LACS app from Facebook
- Open Facebook and go to Settings & privacy → Settings.
- Open Business integrations (or Apps and websites, depending on your Facebook interface).
- Find KVI LACS and remove or revoke access.
After you remove the app, the Facebook connection used by our staff review tool for that authorization will no longer be available.
4. How to request deletion of personal data we hold
To request deletion of personal data we hold from any channel (including website forms, chat, voice, or social), email:
Use the subject line: Data deletion request
Please include:
- Your full name
- The email address and/or phone number you used with us
- Which channel(s) you used (for example: website form, Guru chat, voice, Facebook / Instagram, other)
- Any approximate date or booking/reference details that help us locate your records
We will confirm your request and complete deletion of eligible personal data within 30 days, except where we must retain information for legal, regulatory, security, or clinical record-keeping obligations.
5. Medical records
Medical records are governed by applicable law (including HIPAA where it applies) and are handled through the practice’s medical-records process—not solely through this online deletion form. For medical-records requests, contact the practice at (310) 677-0760.
